Online/Blended Test Form with Roster
The Assessment Center is available for IVCC students who need to take course tests due to accommodations or missed class periods. We are an option for online classes for the class to take their exam in our location as well.
The Assessment Center is based on an APPOINTMENT ONLY schedule. Students must make their appointment 24 hours before they time they hope to come in. Since you have your dates for tests already outlined in your syllabus, please make it a point to strongly encourage students to make their appointments at the beginning of the semester. We do not accept walk-ins.
All exams submitted to the IVCC Assessment Center must also have guidelines (length of time allowed, date of completion, etc.) for testing established by the instructor. Instructors are to complete a test administration form and also provide a roster for online courses. The IVCC Assessment Center is not available to proctor exams for your course in absence of the instructor. Please contact division dean if assistance is needed for such an issue.

Students needing testing accommodations for on-campus testing must make arrangements with the Center for Accessibilty and Neurodiversity (CAN) office.

- Faculty should fill out the Disability Services Testing Form indicating how much time to allow for exams, and whether any extra items like calculators, notes, etc. are allowed.
