The purpose of the Employee Behavior Incident Reporting Form is to allow an employee to make a good faith report without fearing retaliation. The person making a report can remain anonymous. To better help the person making the report, we would need the name of the individual or involved parties. The College will review the form in a timely manner by professionals who will treat the form with respect and discretion. Confidentiality is not guaranteed once the information is shared because the College may have to take appropriate action.
